Manipal Surgeon Breaks New Ground with Shoulder Preservation Surgery Using HDA Augmentation

On Monday, Dr. Prashanth Thonse of Manipal Hospital in Udupi performed a unique shoulder preservation surgery that uses hypodermic dense array (HDA) augmentation to restore joint stability without resorting to arthroplasty. The technique, developed by Dr. Thonse’s research team, integrates a custom-made scaffold that encourages native tissue growth and realigns the joint surfaces.

The procedure involves minimally invasive arthroscopic access, identification and decortication of the damaged cartilage, and placement of the HDA scaffold over the prepared area. Surgeons then secure the scaffold with bioresorbable anchors, allowing the patient’s own cells to populate the structure and gradually replace the damaged tissue. Dr. Thonse explained that initial trials show improved range of motion and a lower risk of implant loosening compared to traditional joint replacement.

This advancement could reduce the need for shoulder replacements among younger and more active patients in the Udupi region, lessen long‑term rehabilitation time, and lower overall health‐care costs.
